Pancreatic necrosis: symptoms and what it is
When a person is struck by any pathology, then all the problems fade into the background.
This is especially true if this pathology is deadly.
It is this disease that is considered to be pancreatic necrosis, which is considered one of the most dangerous pathologies that affect the entire gastrointestinal tract.
It is noteworthy that the disease is quite common among young girls.

What is necrosis
Necrosis - what is it? Pancreatic necrosis is a pathological condition during which there is a change and death of the tissues of the pancreas. Most often, the disease develops against the background of an acute form of pancreatitis.
The pancreas is designed to produce a kind of enzymes that penetrate the intestines and break down fats, proteins and carbohydrates, facilitating the process of food digestion.
When the pancreatic ducts are affected, the enzymes cannot "come out" and begin to digest the gland itself.
Types and forms of pancreatitis
There are a large number of parameters on the basis of which it is possible to classify the disease in question.
They should be discussed in more detail. According to the volume of tissue affected by necrosis, the disease is divided into the following types:
- Local: small-focal, medium-focal and large-focal necrosis.
- Common: total and subtotal necrosis.
Patients who develop mild and medium focal forms of pancreatic necrosis rarely get to the doctor's bench.
Such stages are most often diagnosed during a routine examination. The subtotal stage is characterized by damage to almost all organs, and in the total stage, the organ dies immediately.
According to the rate of progression, pancreatic necrosis is of two types:
- Sluggish.
- Progressive.
According to the presence of foreign cells in the area of the organ under consideration, the pathology is divided into two types:
- Sterile.
- Infectious - when, in addition to enzymes and products of inflammation, harmful microorganisms penetrate into the blood, provoking infectious - toxic shock and sepsis. This phenomenon often ends with the death of the patient.
By the nature of the course, pancreatonecrosis is as follows:
- Hydropic.
- Hemorrhagic.
- Hemostatic.
- Mixed.
Experts can give a positive prognosis only when diagnosing edematous necrotizing pancreatitis.
The causes of the appearance of pathology
The main factor provoking pancreatic necrosis is the acute stage of pancreatitis or exacerbation of its chronic form.
Here it should be borne in mind that pancreatic necrosis is formed in the case of very advanced and severe pancreatitis.
In addition to this factor, there are other reasons for the onset of pathology. They are as follows:
- Passion for alcoholic beverages.
- Abuse of fried and fatty foods that negatively affect the entire digestive tract.
- Presence of stones in the gallbladder.
- Food poisoning.
- Complications after endoscopic examinations of the body.
- Congenital anomalies of the gastrointestinal tract.
- Ulcers of the stomach and intestines.
- Injury to organs during surgery.
- The presence of infections and viruses in the body.
- Infection with parasites.
Less commonly, pancreatic necrosis can manifest itself when the sphincter of Oddi does not work properly, congenital malformations of the pancreas.
All these phenomena provoke the ailment in question, which is considered to be very serious and dangerous.
It should be noted that even a timely started therapy does not give a 100% guarantee of recovery. 40 to 70% of all patients with pancreatic necrosis die.
The main signs of the disease
The symptoms of necrosis include the characteristics of the organism of a particular patient, and any pathological process.
But the main symptom of a developing disease is considered to be painful sensations in the area of the left rib, which radiate to the shoulder or chest.
Necrosis in its manifestations is similar to a heart attack, therefore it is very important to distinguish pathology from one another in a timely manner.
To do this, a person must sit down, raise his knees to his stomach and listen to the sensations: if the pain becomes less intense, then this indicates the presence of pancreatic necrosis.
Frequent symptoms of pathology
There are several characteristic signs that indicate the development of problems with the pancreas.
You should consider them in more detail:
- Soreness. The intensity of sensations can vary depending on the nature of the organ damage and the degree of neglect of the pathology. This indicates that pain is not too severe in all cases. But, despite this, in almost 92% of patients, the sensations are unbearable. In addition, they can be accompanied by cardiac collapse, which is often fatal. The remaining 8% of people have moderate pain.
- Vomiting not associated with food intake and does not relieve the patient's condition. In the masses, bloody blotches and bile can be observed, for the destruction of blood vessels occurs. Incessant vomiting causes dehydration, reduces urine output, urinary retention and increased thirst.
- Great bloating and flatulence. If fermentation is observed in the intestine, then the person feels excessive gas formation. Such a process is dangerous with gas retention and bloating, weakening of organ peristalsis and regular constipation.
- Poisoning the body. With the progression of pancreatic necrosis, intoxication with bacterial toxins is observed. This is manifested by increased weakness and decreased blood pressure, shortness of breath and tachycardia. Toxins can provoke encephalopathy, which is dangerous by the appearance of patient disorientation and coma.
- Hyperemia. When the pathology is severely neglected, the patient's skin turns yellow. In addition, bruising may occur on the abdomen and back due to internal hemorrhage.
- A purulent lesion is diagnosed in the last stages of necrosis. Due to inflammation, the organ increases several times, which causes the appearance of pus and the appearance of toxic hepatitis.
It is worth considering that neglected pancreatic necrosis is practically not amenable to therapy and is considered incurable.
How to diagnose pathology
If a person carefully listens to the signals of his body, he will promptly suspect the onset of the development of pancreatic necrosis and has every chance of a comforting prognosis.
The acute form of pancreatitis can be successfully treated with medications and does not require surgery.
Diagnostic measures that are carried out immediately after hospitalization of a sick person include the following procedures:
- A blood test, required in order to determine the enzymes amylase and lipase, which are capable of breaking down fat.
- Ultrasound examination, which allows you to identify the size, shape and condition of the tissues of the pancreas. In addition, an ultrasound scan helps to examine the location of the affected area and its extent.
- Laparoscopy is used when an emergency diagnosis is required or when the ultrasound results are insufficient to make a diagnosis.
- CT is an examination method that makes it possible to identify affected foci, inflammatory processes, neoplasms and changes in the size of the organ.
- MRI - this technique is considered the most reliable, because with its help one can understand why pain has arisen.
All considered diagnostic measures allow to identify the need for surgical intervention and predict the consequences.
Most often, doctors manage to identify the symptoms and establish a diagnosis at the first stages of the onset of pancreatic necrosis.
In order to speed up the diagnostic process, the doctor must ask the patient whether he drinks alcohol, whether he has liver or gallbladder disease.
Most patients with pancreatic necrosis are admitted to clinics in alcoholic intoxication.
Pathology treatment options
Although statistics do not indicate a positive outcome of treatment measures, the patient must receive decent therapy in order to have a chance for a cure.
Conservative therapy
Drug treatment is often a kind of preparation for surgery or is used to diagnose small focal pancreatic necrosis.
Such therapy should detoxify enzymes, perform detoxification, provide pain relief and eliminate the risk of complications.
For these purposes, it is customary to carry out the following procedures:
- Intravenous saline infusion.
- Treatment with analgesics, antispasmodics, novocaine blockade.
- Taking antibacterial drugs.
- The use of substances that inhibit the action of the proteolytic enzyme.
- Taking antihistamines.
- Conducting symptomatic treatment.
In addition, it is necessary to provide rest to the gland by introducing special nutritional preparations into the patient's body and following a fasting diet for 3-5 days.
During this period, any physical activity is prohibited.
Surgical intervention
If doctors diagnose advanced stages of pathology, then surgery cannot be avoided. Surgical intervention is mandatory if there is an infection in the organ, and if the gland does not respond to drug therapy.
With the development of sterile pancreatic necrosis, an operation is needed when the necrosis diverges to nearby internal organs.
There are 3 types of surgical intervention. You should consider them in more detail:
- Emergency operation carried out immediately after hospitalization of the patient.
- Urgent surgery, which is done within 72 hours if there is no improvement.
- Late surgery prescribed within 2 weeks of admission to the hospital in case of unsuccessful drug treatment.
The situation is complicated by the fact that doctors do not have a single method of performing the operation. Whether a person will recover after it depends only on himself, for he will have to follow a strict diet throughout his life.
Often patients who are faced with pancreatic necrosis suffer from diabetes mellitus, because the body stops producing insulin normally.
Complications of pathology
When the disease progresses, pancreatic enzymes enter the bloodstream and spread throughout the patient's body.
This causes vasodilatation and an increase in their permeability, which leads to the penetration of blood into the space between cells. This phenomenon provokes edema of the organ in question, hemorrhage.
There are a number of consequences that can occur against the background of inflammation and necrosis. They are as follows:
- Parapancreatic infiltration.
- Hemorrhagic effusion of the peritoneal type.
- Pancreatogenic peritonitis.
- The phlegmon behind the peritoneum is aseptic type.
In order to avoid such dangerous consequences, you should promptly seek the help of qualified specialists. Only a doctor can diagnose and prescribe adequate treatment.
Preventive measures
There are several recommendations that will help prevent the appearance of the disease in question and dangerous complications.
They are as follows:
- Compliance with dietary measures when diagnosing pancreatic necrosis. Patients are prohibited from eating fried and fatty foods.
- Refrain from drinking alcohol and other beverages that contain alcohol.
- Leading a healthy lifestyle.
- Correct sleep pattern.
- Treatment of infectious and viral diseases.
In addition, it is impossible to start before the advanced stages of the pathology of the gastrointestinal tract.
Forecast
Doctors cannot give a comforting prognosis for pancreatic necrosis. If its total form was identified, then it is diagnosed only by a pathologist. But even with all other types of the disease, the chance of recovery is negligible.
About 25-50% of patients survive after emergency surgery. In the event that the surgical intervention was performed on the 3rd day, then the chances become even less.
