Stones in the pancreas: causes of stone formation and treatment
The pancreas is an organ that everyone knows. But few people know what pathologies can develop in it. And what he is responsible for in the human body is also not known to everyone. Meanwhile, if the gland does not work properly, then the digestion process does not go as it should. And a person, at best, experiences serious discomfort, at worst, he really suffers. The formation of stones in the pancreas is a process that is easy to start, but not so easy to stop.

What you need to know about the pancreas

Pancreas structure
Pancreas - a small (only 15 cm long), elongated organ, which belongs to the key group in the entire human digestive system. It is located behind the stomach near the duodenum. The gland has only one small duct that connects just to the intestine. And through it, a special pancreatic juice enters it, which is enriched with enzymes involved in the digestion of any food.
On a note! The process of excretion of juice is called exocrine function.
However, the pancreas is also capable of performing endocrine processes. That is, it can produce insulin, glucagon and a number of other hormones that control the processes in the human body. Only these substances do not enter the intestine, but go directly into the blood. Their job is to control glucose metabolic processes.
As for digestion, the pancreas is interesting just from the point of view of the ability to produce pancreatic juice. And it, saturated with enzymes, performs exclusively tasks related to digestion.
Enzymes are substances that are responsible for the breakdown of carbohydrates, fats, proteins. After exposure to enzymes, the prepared components can be easily absorbed in the intestines and used by the body. If they are not broken down, then the bulk of the food eaten by a person will simply pass through the intestines and will not even be absorbed. There will be no sense in such food.
Pancreatic juice contains the following essential enzymes:
- amylase, which can break down carbohydrates;
- lipase, which is responsible for the breakdown of proteins;
- trypsin and chymotrypsin, which are able to break down protein chains into small elements.
Pancreatic enzymes can work only in a slightly alkaline environment, because pancreatic juice also contains bicarbonates that can neutralize hydrochloric acid in the stomach.
On a note! The pancreas works most actively when acids and fats enter the duodenum and irritate its wall. The fastest enzymes are produced in the first 30 minutes after eating. Further, the rate of production decreases, and after about 3 hours, the iron returns to a calm state.

The role of the pancreas in digestion
If the pancreas is affected by any disease, and its cells are damaged, then the process of enzyme formation can be started ahead of schedule. In this case, it develops pancreatitis, from which other pathologies begin to form.
Signs of acute pancreatitis are severe pain in the upper abdomen called colic. When coughing or inhaling, it does not get stronger, which makes it possible to distinguish pancreatitis from appendicitis and other diseases. The attack usually occurs after eating. Also, the signs of pancreatitis include vomiting, nausea. If untreated, pancreatic necrosis may develop - in this case, areas with dead cells appear on the walls of the pancreas.
At chronic pancreatitis the person does not experience severe bouts of pain. But prolonged inflammation of the pancreas (and this is it) can negatively affect the organ and lead to its irreversible changes.
The formation of stones in the pancreas is often associated with developed and progressive pancreatitis. Consider what it is and how to deal with this ailment.
Stones in the pancreas

Presence of stones in the pancreatic ducts
Stones in the pancreas or calculi - this is one of the fairly common complications of a number of chronic pathologies associated with pancreatic disease. Simply put, these are calcifications that form inside the organ and in its ducts, due to which the gland becomes unable to fully or partially perform its functions. Most often, stones begin to appear just as a result of advanced pancreatitis.
Usually, doctors find stones in the duct or in the body of the gland. In her tail, calculi are extremely rare. Stones can be of various shapes, singular or plural.
On a note! In recent years, doctors have noted an upward trend in the incidence of pancreatic stones in patients. But they associate this aspect not with an increase in the incidence of morbidity, but with the emergence of more accurate diagnostic methods. Previously, calculus could only be detected using X-rays, and even then only large stones could be detected. Now CT has appeared. And it gives a chance to find the smallest calcifications.
The process of stone formation in the pancreas is called pancreatolithiasis. Most often middle-aged men suffer from it. The chemical composition of stones is diverse: they contain salts of phosphorus and calcium, aluminum, silicon, magnesium.
The reasons for the development of pathology
There are not so many reasons provoking active pancreatolithiasis. Let's consider them in the table below:

In addition to the reasons listed above, other factors can provoke the appearance of stones in the pancreas, associated with the occurrence of any conditions that are accompanied by stagnation of pancreatic juice in the gland. For example, these include various tumors, cysts, and so on. Stagnation is the main reason why the contents of the pancreas thicken, a sediment forms. The appearance of sediment is accompanied by problems with hormonal and ionic metabolic processes. As a result, the composition of the gland juice changes, the process of calcification begins to occur. And the result is the appearance of stones.
On a note!The presence of ongoing inflammatory processes inside the duodenum, bile ducts, and so on seriously accelerates the process of stone formation.
The result of the appearance of stones is a decrease in the volume of the gland with the same amount of juice produced. Its duct expands, the body stretches, and the pressure inside the organ also becomes higher. Hence - the discomfort and pain that a person feels. Lack of treatment can provoke a decrease in the amount of enzymes a person needs, which, in turn, causes the development of secondary diabetes mellitus.
Symptoms of the disease
How to understand that stones have appeared inside the pancreas? There are undoubtedly signs of pathology, but the manifestation of the disease will largely depend on the size of calculi, their number, location, and so on.
For example, if the stones are small and present in small quantities, then the patient may not even be aware of their presence and the processes of pancreatolithiasis. Such stones are usually found on ultrasound.
More obvious signs of the appearance of stones are as follows:
- pain in the stomach, especially clearly felt after eating spicy or fatty foods;
- burning pains;
- pain after drinking alcohol;
- decreased appetite;
- nausea and occasional vomiting, sometimes with bile;
- stool problems: it usually becomes irregular;
- an increase in the amount of saliva secreted;
- painful sensations on palpation of the abdomen in the epigastrium;
- other digestive disorders.
All these signs can be observed in the presence of other diseases of the digestive system. And this is the main difficulty of accurate diagnosis of pathology without conducting certain studies. Unfortunately, there are no specific signs indicating the formation and presence of stones in the pancreas.
Attention! If the stone manages to migrate and block the bile duct, then it will unwittingly block the outflow of bile from the gallbladder. In this case, another symptom appears - jaundice. The urine becomes a bright yellow color, and the bowel movements are light gray. In a word, all the signs of the so-called obstructive jaundice are noted.
The formation of calculi can be accompanied by the development of chronic pancreatitis, the appearance of abscesses and cysts inside the pancreas, inflammation of the peripancreatic tissue, and even bleeding. This is how complications can manifest.
Diagnosis of pancreatolithiasis

Retrograde cholangiopancreatography procedure
Diagnosis in case of suspicion of the presence of stones in the pancreas is quite simple from the point of view of medicine. Another question is if doctors do not even suspect about the presence of stones. Then it is not so easy to identify them. But if there are already suspicions, then the doctor will direct the patient to take all the necessary tests and undergo a number of examinations.
Attention! If a person undergoes an X-ray of the abdominal cavity, but due to other diseases, then the doctor may not pay attention to the presence of stones in the pancreas. Radiography shows stones as small shadows.
If the indication for an X-ray was precisely the suspicion of the presence of stones in the pancreas, then this study will be enough to make a diagnosis. It is advisable to take several pictures in different projections. As a clarifying study, the doctor can refer the patient to ultrasonography, CT, MRI. Such techniques will allow not only to identify stones, but also to find out where they are located. And they will also show in what state the tissues of the gland itself are at the moment. You can also carry out retrograde cholangiopancreatography, which will accurately show where the calculus is located, and also help to find out how far the gland duct is passable.
Pathology treatment

Operation by laparotomy
After the diagnosis, the doctor will definitely prescribe treatment. The chosen method of therapy will depend on the results of the research: the number of detected stones, their size, location, patient's condition, etc. So, the primary goal of treatment is to eliminate inflammatory processes inside the gland, as well as restore the normal metabolism of elements such as phosphorus and calcium. It is necessary to eliminate the swelling of the tissues of the gland.
The doctor will prescribe the so-called substitution therapy and a specific diet. Soon, the patient will feel significant improvements. A number of small stones from the gland will be able to leave the organ on its own and go into the intestines.
However, if the patient has sufficiently large stones, then such treatment will be practically ineffective. And then the patient will have to go under the surgical knife. But you shouldn't panic and be afraid ahead of time. Now there are so-called minimally invasive methods for removing calculi. These include, for example, the endoscopic method for removing stones from the main duct of the gland. Such operations are much more easily tolerated by patients. And the rehabilitation period after them is quite short. But, unfortunately, endoscopic operations do not always allow removing all calculi completely.
On a note! People with stones in the pancreas are advised to seek the advice of an abdominal surgeon.
In the presence of a large number of stones, the patient will be recommended laparotomy and pancreatomy. They are also relevant if the stones are localized in the area of the finished part of the gland or deep inside it. This type of surgical intervention is more complicated than the previous one. Complications can occur after surgery. But still, if there are certain indications, you need to decide on it.
It is important to understand that stones that are in the pancreas for a long time can provoke a serious deterioration in the condition of this organ. Including they can lead to the appearance oncological diseases. So it is necessary to get rid of them or, at least, be monitored by a doctor. Conservative therapy is necessary even when identifying the smallest calculi that do not yet cause discomfort and pain. Disease prevention traditionally includes proper nutrition and giving up bad habits.
